REQUIREMENTS

  1. Dully filled introductory form, signed and endorsed by the Head teacher. (Attached)
  2. An athlete’s full photography taken with the Head teacher and endorsed by the Head teacher at the back. ( No group Photo)
  3. Photocopy of birth Certificate Endorsed by the Head teacher.
